Purpose: The Regional Director of Nursing and Partnerships is responsible for leading the development, implementation and evaluation of clinical policies of Grace Healthcare Services hospice care as well as managing all aspects of the interdisciplinary team and quality initiatives. Serves as the primary operations and clinical point of contact for the partnership with the hospitals.

Key Responsibilities:

This position has supervisory duties

· Participate in the coordination, planning and delivery of quality and compliant hospice care.

· Evaluate patient services provided by the interdisciplinary team (IDT).

· Review care plans and medication lists for new admissions and review charts.

· Maintain working knowledge of hospice state licensure as well as federal regulations.

· Monitor services and documentation to meet hospice policies, state and federal regulations.

· Serve as liaison to VP of Clinical Services regarding challenges and patient care.

· Supervise and participate in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team including any contracted staff.

· Be available on-call if needed. Provide support for on-call staff.

· Develop physician relationships to familiarize them with hospice care.

· Act as liaison between IDT members, patients, their families, and our customers.

· Contribute to effective patient and customer relationships by assisting patients, families, physicians, and staff to resolve expressed concerns.

· Facilitate interdepartmental communication with co-workers and staff.

· Participate in business development activities such as implementations and presentations.

· Responsible for assisting the program to meet and/or exceed all program goals.

· Responsible for compliance of all team members notes including accuracy and timeliness of all patient related documentation in HCHB.

· Responsible for reviewing all team members' schedules and time and activity sheets.

· Mentor staff on the team and monitor performance of team, complete staff performance evaluations. Work with Human Resources on any employee coaching/counseling.

· Manage audit list for compliance and minimal exposur


Qualifications:

· NJ Registered Nurse License required.

· MSN preferred - BSN required

· 3-5 years' experience preferably in hospice or home care, with progressive management responsibilities

· Excellent communication and interpersonal skills.

· Ability to commute throughout the state of NJ
